What personal data we collect and why we collect it

We ask for certain information such as your username, real name, birth date, address, phone number, company,e-mail address,details regarding your health or the health of the person seeking services from Housecall Practitioner Services ( “us”, “we”, “our”), and other pertinent information that may be required to process an application for patient services and or health membership with Housecall Practitioner Services when you register an account and or apply for a membership with us  or if you correspond with us. We may also retain any messages you send through this website (“site”), and may collect information you provide in a post and or comment to us. We use this information to operate, maintain, and provide to you the features and functionality (“Services”, “Service”) provided by us. Your username and if you choose to disclose it and real name may be published publicly. Once published your username and / or real name may not be able to be removed.

Comments

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

Cookies

  • Who can see User Content: Any User Content that you voluntarily disclose for posting to this site becomes available to the public and cannot be removed, except by us at our sole discretion. Once posted on this site, certain User Content may not be removable from public view, as copies may remain viewable in cached and archived pages of this site, on other sites that have republished us, or if other Users have copied or saved that information. If you do not wish your User Content to be available to the public, please don’t post it on this site. Your username may be published publicly. Once published, your username may not be able to be removed.
  • Who can see your IP address: If you post User Content on this site while not logged in, your IP address will be published with your User Content in order to attribute your contribution to this site. A selection of Trusted Users with administrative powers on this site have the ability to review IP addresses of users to reduce abusive behavior. How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What rights you have over your data
